Website privacy policy

This policy covers centralhealthec.com. It is separate from the HIPAA Notice of Privacy Practices below, which covers your health information as a patient.
What we collect
If you use the contact form, we receive what you type: your name, email, phone number, and message. It is emailed to the office and used only to reply to you. We use Google Analytics to understand how people use the site, which collects anonymous usage data through cookies. We do not sell any information.
Scheduling and forms are on other sites
Online scheduling runs on SKED and new patient forms run on ChiroHD. When you click through to either, you leave this site and their privacy policies apply. Both are services the practice uses to run the office, and the information you enter there is treated as patient information under HIPAA.
Cookies
Analytics cookies can be blocked in your browser settings or with the Google Analytics opt-out add-on. The site works without them.
Children
This site is not directed at children under 13, and we do not knowingly collect information from them online.

How we use and share your health information
We use your health information to treat you, to bill for that care, and to run the office. For example, we may send records to another provider involved in your care, send a claim to your insurance company, or use your information to schedule visits and review the quality of our care. Other uses required or permitted by law, such as public health reporting, are listed in the full notice. Anything else requires your written authorization, which you can revoke at any time.

Our duties
We are required by law to keep your health information private, to give you this notice, and to follow it. If we change it, the new notice will be posted here and in the office.
Complaints
If you believe your privacy rights have been violated, you can file a complaint with the practice at the address above or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
Nondiscrimination notice
Central Health complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, or sex. We provide free aids and services to people with disabilities, such as written information in other formats, and free language services to people whose primary language is not English, such as qualified interpreters. If you need these services, call (715) 514-1666.
If you believe we have failed to provide these services or discriminated in another way, you can file a grievance with the practice in person, by mail, or by phone. You can also file a civil rights complaint with the U.S. Department of Health and Human Services Office for Civil Rights.
Accessibility statement
We want everyone to be able to use this site. It is built to meet WCAG 2.2 Level AA: readable type sizes, strong contrast, keyboard navigation, and text alternatives for images. Terms of use
This site is for general information about Central Health. Sending a message through the contact form does not make you a patient and is not the right way to reach us in an emergency. Content on the site belongs to Central Health and may not be reused without permission. Scheduling and intake are handled by separate services under their own terms. We do our best to keep everything accurate but do not guarantee it, and we may update these terms at any time. Wisconsin law applies.
